Do you have a stable internet connection at home with a reasonable speed of transmission
(e.g., you are able to watch videos on the internet)?
Do you have a desktop or laptop computer? If not, do you have a tablet that is connected
to the internet (e.g., an i-pad)?
Does your computer or your tablet have a camera, a microphone, and a speaker?
Can you use your computer or table while being in a quiet, private space where you
can work uninterrupted? In other words, during your evaluation, can you be in a private
room where you can close your door and not have others walk by, interrupt you, etc?
Similarly, if you have pets at home, can you keep them away from your workspace during
the evaluation?
Making a decision:If you think an evaluation at the CCC may be for you, please send an email to matt.euler@psych.utah.edu, with the following information:
Your name and the best way to reach you (email, or phone and best time of day)
A very brief, 2-3 sentence description of your concerns and the reason you would like
to be seen.
Please also note that email is not a secure means of communication.
Someone from our clinic will get back to you shortly to complete a phone-screening.
If your responses indicate that the CCC is not for you, we will provide you with information
about other available resources.
Clinic Fee: Comprehensive cognitive screenings are $50, paid prior to your appointment via check. Please allow one week for the check to
arrive. Screenings include: interview and testing, scoring and report, and one 45-minute,
feedback session. For those clients for whom the clinic fee is prohibitively high,
the fee may be reduced depending on specific circumstances. Sorry, we cannot accept
insurance.
